SVC STAR Award Nomination form - DUE Wed. March 6, 2024
The SVC STAR Award recognizes FAS employees for sustained, exceptional performance and significant contributions benefiting UCSF values and priorities, including contributions to the FAS True North which is perfectly aligned to support UCSF excellence. This award includes an achievement-level cash reward per the Campus STAR program guidelines.
Some specifics
- The SVC STAR Achievement Award will be presented at the June FAS Managers’ Town Hall.
- This award includes a $5000 achievement level reward (up to 10% of salary) per the Campus STAR program guidelines.
- As many as four (4) nominations will be selected per year to preserve a high level of prestige.
- The intent is to honor and recognize exceptional performance and significant contributions that are sustained over an extended period of time (a minimum of six months), with at least a portion occurring in the current fiscal year.
- Departments should consider all levels of staff and are encouraged to submit at least one nomination in recognition of an individual contributor (P3 or below).
Process for FY24
Each FAS department (FET level) can nominate up to two employees, preferably in each category (P3 and below and CX; and P4 and above). Due by the end of the day Wednesday, March 6. Nominations will be vetted by the SVC STAR Committee and a recommendation will be sent to FET via email.

What do you need to submit an SVC STAR Award nomination?

- New for 2024: Complete additional True North statement here answering criteria 5: Benefit of Contribution to FAS True North – How did the nominee’s contribution substantially benefit the FAS True North Pillars (Our People, Customer Experience, Safety, Resource Management & Stewardship, Innovation)?
Provide a brief description of the specific reason for nomination for an SVC STAR Award (download nomination form):
- Time – For what period of time was the nominee’s effort sustained to perform exceptionally (minimum of six months is required for an achievement level award)?
- Base of Operation – Assess the nominee’s contribution in proportion to their position’s capacity to make an impact. (Some employees in higher level positions/jobs may have a greater capacity to contribute more visibly than those in lower level positions). =
- Exceptional Performance – How did the nominee demonstrate exceptional performance that consistently exceeded goals and work expectations in quantity and/or quality?
- Benefit of Contribution to Chancellor Goals or PRIDE Values – How did the nominee’s contribution substantially benefit the Chancellor’s four goals (Foster Innovation; Build value-added partnerships; Bolster financial resiliency; Nurture UCSF’s culture and empower its people) and/or Pride Values Professionalism, Respect, Integrity, Diversity and Excellence.
- Benefit of Contribution to FAS True North – How did the nominee’s contribution substantially benefit the FAS True North Pillars (Our People, Customer Experience, Safety, Resource Management & Stewardship, Innovation)?

- Award payout will be less usual individual taxes
- Cumulative awards of over $5k in a year require advance HR approval.
- Recipients cannot receive more than one award for the same contribution.
- For contributions spanning shorter durations, consider other recognition programs including STAR SPOT awards, recognize.ucsf.edu or non-cash awards such as gift cards up to $7
